Use this form to tell us about any changes to your circumstances that may affect the type of travel assistance you get. This could include your child moving address or school, a change to your child’s needs or change in your employment status.
You will need
The information you need for the form will depend on the change you are reporting.
Your new address, if you are moving out of Ealing borough (if known)
New school address, if your child is moving schools (if known)
New medical information, if you are reporting a change to your child’s needs
Bank details, if these have changed.
Uploading supporting documents
All supporting documents must be up to date (no less than a year old).
We will ask you to upload some documents. For example, new medical information supported by a medical document, new school details, up to date bank details.
Accepted file types: Word (docx), Acrobat (pdf), Excel (xlsx), Plain text (txt)
Zip files can't be uploaded
We also accept photos taken on phones and tablets and the following image file types - jpeg, gif, png
There is a maximum individual file size per document of 50MB.
What happens next
Once we review the application, you will get a decision within 10 working days to tell you if your transport arrangement will change.
